Conveniently located on the border of Queens and Long Island with Oceanfront views, Brookhaven Rehabilitation & Health Care Center is a warm and caring skilled nursing facility, which specializes in Short-Term, Long-Term, and Bariatric weight loss services.

Our 298-bed facility offers a variety of nursing services in a friendly and comfortable environment. Our profession staff recognizes that every resident is unique and special and is committed to developing treatment plans that meet the physical and psychosocial needs of each resident to promote optimal well-being and a sense of independence.

At Brookhaven, Board Certified physicians, and licensed nurses offer specialized care to all residents 24/7. Our experienced rehabilitation specialists work closely with each individual, while our dieticians create individualized meal plans that suit every resident’s needs.

In the past, Brookhaven Rehabilitation & Health Care Center was featured on TLC in a documentary-reality show series, taking the audience on a personal and introspective journey through the lives of some of our bariatric patients.
