Admission to Brookhaven is a seamless process, with our experienced staff and social workers assisting residents and their family to make a smooth transition.

Whether a resident is being admitted for a short or long-term stay, our staff works to anticipate their needs and assist them in every way possible as they join the Brookhaven family.

We strive for all our patient to be aware of and receive all health care benefits that are available to them. We work with Medicare, Medicaid, and most managed care or private insurance companies to coordinate care services.

Interested in Referring Someone in the Community?

If you are interested in admitting a family member directly from the community, they have to be New York State resident and have a participating insurance company.

If your insurance company is in network with our facility, you can get an application (PRI & Screen) from a certified Nurse from any local Home Healthcare Agency (ie: Visiting Nurse Service). Once you complete those two forms, have the nurse fax them to our admissions office: Fax#: (718)327-5506.

Once the application is reviewed by our clinical team, if the application is approved you should be contacted back within three business days. If you do not receive a call by then, please call the admissions office to verify that your application was received.
